Required Qualifications:

  • Graduate of an accredited school of professional nursing program with a current license as a Registered Nurse in the state of Minnesota.
  • Minimum of two (2) years of nursing experience in post-acute (long-term care, assisted living, memory care or home care)
  • Previous supervisory/management experience in a health care setting

 

Preferred Qualifications:

  • CPR certified
  • Bachelor’s degree in nursing preferred

 

About the Job:
The Director of Healthcare Services/Director of Nursing (AL) is a member of the key leadership team at The Homestead at Coon Rapids. The position is primarily responsible for all clinical care and services provided to assisted living residents and home care clients. High quality care, customer service and team member satisfaction are three critical responsibilities of this role.

 

Essentials:
Administration

  • Participates in campus wide strategic planning activities
  • Partners with the Licensed Assisted Living Director to assure AL residents receive high quality care and services
  • Partners with the Executive Director for the development and growth of the campus based home care agency
  • Assures compliance with all state, federal and local laws pertaining to assisted living and home care
  • Provides leadership to all clinical assisted living and home care staff
  • Responsible to assure staff education and monitors for compliance
  • Reviews policy and procedures annually
  • Recommends new policy and procedures or changes to current policies to improve quality of care, meet state regulations and assure efficiency and effectiveness of services
  • Assists in planning and development of assisted living and home care annual budgets
  • Responsible for risk management and completion of incident reports
  • Reviews all incidents involving assisted living or home care residents and is responsible for investigation and timely notification of the Licensed Assisted Living Director and state officials according to the Minnesota’s Vulnerable Adult Protection Act
  • Provides support to the facility by being available for, or assuring nursing availability, for on call services
  • Provide staff direction through general meetings, timely personnel evaluations and productivity expectations
  • Ensures accuracy in documentation and appropriate billing for all care and services

 

Coordination of Services:

  • Introduces home care services to new clients and families
  • Conducts pre-admission screening of prospective residents and home care clients
  • Performs initial and ongoing assessment of physical & psychosocial needs and coordinates with other

 

departments, agencies and care providers as needed

  • Maintains ongoing communication with families, physicians and other care providers regarding resident or client status including condition changes, reaction to drugs, treatments or other incidents
  • Partners with residents and clients to develop a plan of person centered care/services
  • Ensures/coordinates necessary ancillary services and appointment scheduling to ensure that needs are met
  • Ensures appropriate documentation in the resident or client record
  • Initiates a resident or client service plan according to their individualized needs and as prescribed by Physician, medical provider and/or VOANS policy
  • Provides resident/client and significant others education on conditions and treatments

 

Coordination of Services (Assisted Living and Home Care):

  • Participates in hospital discharge care conferences to provide assisted living and home care expertise for resident or client admission/readmission to The Homestead at Rochester as needed
  • Introduce home care service to clients and families for all new referrals
  • Coordinate services to meet resident or client needs from the time of the new referral through the admission to services.
  • Establish and implement priorities for services when resource limitations require
  • Provide consultation to Home Health Agency staff regarding new home care or assisted living nursing treatments.

 

Quality Improvement:

  • Establishes and leads quality improvement planning teams
  • Identifies data collection requirements and reports to evaluate the effectiveness of the programs and to

 

ensure quality of client/resident care

  • Identifies systems to recognize resident/client needs, respond to resident/client needs, and to measure the outcomes of nursing interventions. Utilizes this data to provide direction for improvements
  • Provides training, supervision and monitoring of staff in the administration of medications, to include frequently auditing medication sheets
  • Supervises the maintenance of resident charts, review documentation performed by care giving staff.

 

Human Resources

  • Makes health care staff hiring decisions for assisted living and home care
  • Participates in orientation and ongoing training of team members
  • Ensures staff development, in-service education and online training programs are completed to ensure competency of staff and to meet regulatory requirements
  • Provides staff direction through general meetings, timely personnel evaluations and identified performance expectations
  • Builds a strong team with complimentary skills
  • Communicates effectively with clinical team and all campus team members
  • Ensures that corrective action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion or disability or other protected class
  • Ensures current licensure/certification and competency of all clinical staff
  • Attends required in-services & completes assignments online modules

 

Customer Services:

  • Possesses a strong customer service orientation in nursing practice, care and services
  • Implements programs to ensure excellent customer service in assisted living and home care
  • Models and holds staff accountable for the achievement of high quality customer services
  • Participates bi-annually in facilitating resident, client and family satisfaction survey and implements desired or necessary improvements
  • Follow ups with all resident, client and family issues or concerns and keeps Assisted Living Director and/or Executive
