Position Summary:


Rutgers, The State University of New Jersey is seeking a Patient Navigator for NJMS-Emergency Medicine. The Patient Navigator supports various aspects of the ED ALT project funded by SAMHSA. The position will devote 100% of its efforts to the ED ALT project until the expected end date of September 29, 2026.
Among the key duties of this position are the following:
  • Meets patients in person when they are diagnosed with painful conditions in the ED/Observation setting within the University Hospital
  • Addresses early recovery needs by providing educational information, resources, and linkage to appropriate clinical and social services. Particular emphasis will be placed on linking individuals diagnosed with opioid use disorder (OUD) to medication for opioid use disorder (MOUD).
  • Trains ED providers and patients with painful conditions on alternative nonpharmacological interventions such as breathing exercises, meditation, virtual reality, and music therapy
  • Gathers information for determining health and wellness dimension needs, including housing, employment, and education, and uses this information to identify services that may benefit the referred patients.
  • Participates in patient recruitment for services by reviewing medical charts and communicating with physicians and APNs in the ED/Observation Unit.
  • Provides patients diagnosed with painful conditions and OUD in the ED/Observation Unit with linkage to addiction care and social support services
  • Gathers, organizes, and analyzes data requested by Principal Investigator and Project Director to meet specific project goals.
  • Serves as a backup for other research/service grants within Emergency Medicine that utilize navigators or similar positions.

Minimum Education and Experience:

  • Bachelor's Degree in social work or Human Services plus a minimum of two (2) years of fulltime professional experience working with patients diagnosed with OUD or SUD.
  • Equivalent education, experience, and/or training may be substituted for the degree requirement.

PHYSICAL DEMANDS:
  • Ability to speak clearly and write proficiently.
  • Hearing is essential.
  • Must be able to focus on the task at hand despite numerous interruptions.
  • Must be able to sit for an extended period and document into an electronic medical record.

WORK ENVIRONMENT:
  • The position requires working in a moderate noise office setting. Fast-paced, high-pressure office environment; must be able to cooperate with a broad spectrum of staff and site visits in the University Hospital's Emergency and Observation Department.
