Position Overview:

The ICM Registered Nurse will work with a multidisciplinary team to provide individualized person-centered services to adults with a severe mental illness. This is a community-based program with flexibility to go into the community or client’s home to provide care. The work includes individual case management, groups counseling, family support group, skills training, and coordination with medical providers to assist the client in achieving identified goals, improve their health and wellness and reducing hospital and jails stays.

Essential Job Functions:

Main Duties

  • Primary responsibility for the development of individualized nursing assessments on all program participants. Develop, implement, and evaluate the nursing regimen for recipients.
  • Participate in implementing and maintaining nursing standards
  • Participate in determining resources and policies essential to the delivery of nursing care services
  • Assess medical care needs and other services to support wellness
  • Recommend health objectives for recipients’ service plans
  • Facilitate consumer education on prevention, health, wellness, diet, HIV/AIDS education, smoking cessation and recovery
  • During the assessment obtains information: a) to establish the degree of medical risk, medical care and interventions required to promote positive health outcomes, and b) to lead team in primary care issues
  • Provide education to ICM staff on physical health issues and medical care protocols
  • Act as advocate and liaison for recipients in securing medical care, and other community supports that promote integrated physical and mental health wellness
  • Review and audit medication management procedures and teach all ICM staff how to follow and adhere to the procedures
  • Coordinate ICM team medical care in coordination with LMP
  • Ensure all consumers have primary care provider and receive annual physical and dental examinations and maintain prescription schedule
  • Provide individual health counseling as needed
  • Administer IM’s and vaccines, plans and reads PPD’s for ICM clients as well as other individuals enrolled in Sequoia Mental Health Services.
  • Transcribe orders and updates Medication Administration Records
  • Monitor vital signs and side effects of medications and reports findings to Team
  • Evaluate, on a continuing basis, nursing intervention and expected outcomes
  • Involve family, significant others, landlord and housing providers in service provision
  • Build in review and evaluation points, and modifies the service plan in the light of feedback from recipients and relevant others

Required Skills:

  • Registered Nurse (RN) with current Oregon State license
  • At least three years of experience working with adults with serious mental illness and co-occurring substance use disorders
  • Experience understanding various models and theories pertaining to treatment of serious mental illness and risk management
  • Extensive knowledge of healthcare, community treatment, support services and resources
  • Ability to work effectively with diverse team and remain highly organized and self-motivated
  • Proficient in computers and fluency in Spanish preferred
  • Excellent communication skills
