REPORTS TO: Executive Director, Home Health and Hospice Services

SUPERVISES: Clinical and Administrative Team Members

POSITION SUMMARY:

The Regional Director is responsible for maintaining the highest quality of clinical services, including overall day-to-day branch operations. Responsible for implementing the delivery of quality patient care. The Regional Director has responsibility and authority for maintaining quality patient care, in accordance with the organization’s objectives and policies, through planning, coordinating, implementing, and evaluating the service.

 


PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS
:

  • Facilitates and coordinates all home care services.
  • Provide leadership, oversee clinical responsibility of professional and paraprofessional Personnel.
  • Maintains clinical skills and current working knowledge of home health regulations.
  • Complies with accepted ethical conduct and professional standards of nursing practice as set forth by American Nursing Association, the Department of Public Health, and other regulatory commissions.
  • Responsible for oversight of the patient acceptance process.
  • Responsible for recruitment, hiring, orientation, and evaluation of clinical and non-clinical team members in conjunction with the Department of People and Culture.
  • Responsible for identifying Team Member development needs and assures continuing education is maintained.
  • Performs Annual Evaluations and competencies on direct reports.
  • Develops written goals based on individual and department needs as evidenced by the performance evaluation.
  • Manages and evaluates services provided by Team Members through a variety of means such as home visits, conferences, record review, etc. Provides feedback to Team Members to enhance performance.
  • Partner with the People & Culture team to ensure consistency of expectations, the following of SOP’s, and coordination of people leadership to align with organizational expectations.
  • Facilitates and conducts individual and interdisciplinary patient conferences to achieve therapeutic, restorative, health promotion, preventative, and rehabilitative goals for each patient.
  • Demonstrates ability to identify and resolve interpersonal conflicts constructively when dealing with Team Member members of the organization.
  • Assists in developing programs to meet the needs of the community.
  • Assesses staffing patterns according to patient and organizational needs and collaborates with Executive Director to develop staffing plan.
  • Coordinates with the OVNA management team to ensure adequate support and compliance with agency application usage policy and procedures. Participates in developing and revising organization policies and procedures and programs and educates Team Members to ensure implementation and follow through.
  • Responsible for quality improvement initiatives, safety activities and HIPAA compliance.
  • Acts as a regional resource, advocate and facilitator to maintain continuous compliance to meet all regulatory standards.
  • Continuously approaches responsibilities with a positive attitude according to the standards of nursing practice.
  • Assures medical record claims integrity and clinical claims oversight.
  • Performs duties of a registered nurse.
  • Other responsibilities as assigned by the Executive Director of Home Health and Hospice Services.

 


SUPERVISORY RESPONSIBILITIES

  • Clinical Team Managers.
  • Direct Care Personnel.
  • Office/Clerical Team Members. 
  • Responsible for assisting with the implementation and monitoring of the organization’s quality assessment performance improvement (QAPI) program as directed by the Director of QAPI
    • Ensures that there are processes in place to monitor and measure all activities related to QAPI.
    • Provides direction and coordination of quality improvement activities utilizing continuous quality improvement principles and methodology.
    • Coordinates activities related to clinical record review for data collection.
    • Compiles, trends, and reports quality data in the following areas: Patient care, safety, risk management, infection control, outcomes, customer satisfaction, Team Member retention, and key financial information.
    • Works collaboratively with other OVNA leadership and clinical team managers to assure that documentation is complete and consistent with care and reflects legal requirements.
    • Guides appropriate follow up while collaborating with clinical team managers to follow-up on concerns expressed by patients, families, and physicians. 
  • Responsible for ensuring processes to monitor and evaluate safety, risk management and infection control programs.
    • Coordinates, educates and implements monitoring activities for safety, risk management and infection control.
    • Coordinates, educates and implements standard practices set forth for OVNA in compliance with Conditions of Participation and Medicare reimbursement guidelines.
    • Coordinates mandatory in-service programs on OSHA regulations for safety and infection control.
    • Acts as resource to staff in identifying safety and risk management issues for patients and staff.
    • Reviews all incident reports for completeness, risk, standard of care determination and appropriate follow up. Maintains a log for trending and makes recommendations related to trends identified.
    • Maintains current knowledge related to home care safety and infection control standards.

 

  • Collaborates with QAPI Director, other Regional Directors and Clinical Managers to provide a process to promote and ensure Team Member orientation, competency and in-service education programs.
    • Assists in identification of skills for annual competence and provides in-service training support.
    • Maintains aggregate records of Team Member orientation and annual competency requirements as part of the QAPI plan.
  • Monitor organization compliance with all applicable regulatory standards.
    • Maintains current knowledge related to Home Health Conditions of Participation to ensure that OVNA is compliant with all state and federal guidelines.
    • Responsible for ensuring policies and procedures reflect all related local, state, and federal regulations and makes policy recommendations for policy changes to leadership at the OVNA and the governing body.
    • Acts as resource to clinical team managers regarding standards, policies, regulations and provides in-service training to leadership and staff as needed in these areas.
  • Acts as a clinical resource to staff.
    • Maintains updated nursing knowledge and skills
    • Assists with patient visits as needed.
    • Assists with administrative call on a rotational basis and clinical call as needed.
  • Provides assistance to community education team in meeting community education needs.
    • Maintains positive relationships with referral sources.
    • Maintain positive energy in the community.
    • Acts, when needed, as the OVNA representative at community meetings, civic and professional organizations as needed.
  • Promote self-growth and collegial relationships with others in the home health industry.
    • Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal and professional networks and participating in professional organizations.

QUALIFICATIONS:

  • Bachelor’s degree in Nursing - required
  • Master’s Degree - preferred
  • Registered Nurse with current licensure to practice professional nursing in the State of Massachusetts - required
  • Minimum of 3 to 5 years’ experience as a Department/Regional Management at Associate Director or Director Level - required
  • Minimum 5 years experience serving in a Homecare setting - required
  • Minimum 5 to 7 years’ experience years of Homecare Clinical Nursing - required
  • Self-directed and able to work with minimal supervision.
  • Possess excellent coordination and communication skills.
  • CPR Certification – required

 


PHYSICAL REQUIREMENTS/WORKING CONDITIONS
:

Primary work will be performed indoors in an office setting, may be exposed to the elements (heat, cold, rain, snow); walking and standing frequently; occasional lifting of up to 25 pounds.
