Summary

The primary responsibility of this key clinical pharmacist specialist is to provide clinical care to pediatric patients with hemophilia, thrombotic disorders, and other benign hematological diseases. This position reports to the inpatient hematology/oncology pharmacist manager.

Responsibilities*

To provide direct patient care, to assist in the development, implementation, provision, and monitoring of pharmacy services for the Inpatient Pediatric Hematology Service, Pediatric Hematology Consult Service and Pediatric Hemophilia/Thrombophilia Ambulatory Clinics.

An inpatient/outpatient pediatric hematology clinical pharmacist specialist would:

  • Round with the inpatient pediatric hematology service (primary service) daily and serve as a resource and liaison for the inpatient pediatric hematology consult service [70% inpatient effort]
  • Establish a presence in the pediatric hemophilia and thrombophilia clinics (two to three half days per week) [30% outpatient effort]
  • Provide clinical care and service to patients with hemophilia, thrombotic disorders, immune thrombocytopenia purpura, sickle cell pain crises, and other non-malignant hematology disorders.
  • Assist in the development and implementation of new pharmacy services as it relates to non-malignant hematology.
  • Collaborate with the adult hematology consult pharmacist specialists on system-wide anticoagulation services and immunotherapies.
  • Partner with the pediatric hematology/oncology pharmacist specialists on patient care, research, and educational initiatives

The inpatient/outpatient pediatric hematology clinical pharmacist specialist would provide organizational value by:

  • Improving medication safety and continuity of care for patients receiving blood factor products, immunotherapy, and anticoagulants.
  • Creating and revising medication use guidelines for high-cost hematology agents (ex: anticoagulants, blood factor products, biotherapy, immunotherapy)
  • Reducing requests to leadership for high-cost, non-formulary medications
  • Performing independent and collaborative clinic visits. G-code billing will be utilized for independent visits.
  • Leading outmigration initiatives to decrease inpatient bed days for pediatric hematology population and increase revenue by administering high-cost therapies in the outpatient setting.
  • After establishing this new service, actively participating in the training of hematology/oncology fellows, medical residents, medical students, pharmacy residents, pharmacy students, advanced practice providers and nurses as other clinical pharmacists across the system.
  • Participate on appropriate Department, University, and Health System committees.
  • Conduct or participate in research, drug use projects, cost reduction/management initiatives, and quality improvement activities consistent with the University of Michigan Health System’s goals and objectives.

Required Qualifications*

Education:

  • A Doctor of Pharmacy degree or an equivalent combination of education and experience is necessary.
  • Completion of two years of pharmacy residency or an equivalent combination of education, training, and experience is preferred.
  • Board of Pharmacy Specialties certification is preferred.
  • Excellent communication skills (fluency in writing, reading, and speaking English).

Licensure & Certification

  • Current license to practice as a Pharmacist in the State of Michigan
  • Possession of a state narcotics license is necessary

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third-party administrator to conduct background screenings.  Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
