Summary of Position

The Salvation Army Northern Division is hiring a Service Coordinator. The major function of the service coordinator is to provide general case management (including intake) and referral services to all residents needing such assistance. They may also provide formal case management (i.e., evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident’s situation and needs) for a resident when such service is not available through the general community.

This is a regular full time position with a work week expectation of 40 hours per week. Expected pay for this position is $20-$22.50 per hour depending on experience and qualifications.

Qualifications

  • Bachelor’s degree of Social Work or degree in Psychology or Counseling is preferable; a college degree is fully acceptable. However, individuals without a degree, but with appropriate work experience may be hired.
  • Training in developmental processes and needs, youth services, disability services, eligibility for and procedures of Federal and applicable State entitlement programs, legal liability issues relating to providing service coordination, drug and alcohol use and abuse by the youth, and mental health issues.

* This requirement is not a prerequisite for hiring. IF not certified upon hire it must be accomplished and certified within one year of employment.

  • Two to Three years experience in social service delivery with youth and youth with disabilities.
  • Demonstrated working knowledge of supportive services and other resources for youth and youth with disabilities in the area served by the project.
  • Demonstrated ability to advocate, organize, problem-solve and provide results for the youth and disabled served.
  • Experience working in a social service setting preferred.
  • Understanding of the delivery of Social Services internally and community wide.
  • Ability to accept supervision and work with a team of co-workers.
  • Ability to work well with clients of diverse cultural and socioeconomic groups, respect confidentiality, able to work within ethical and legal parameters of social services.
  • Willingness to support the philosophy, goals, mission, and objectives of The Salvation Army.
  • Must pass the background checks.
  • Must have the ability to operate a computer, use a keyboard to enter information and enter client information into ServicePoint database.
  • It must be understood by the incumbent that the provisions of Safe From Harm will apply upon commencement of employment and also understood that the required Safe From Harm training be satisfactorily completed within the first thirty (30) days of employment.

Essential Duties and Responsibilities

  • Keep accurate case notes of phone calls, interviews and documentation in ServicePoint. Data entry in ServicePoint and kept current with database use as well as computer updating as requested.
  • Attend educational opportunities when appropriate.
  • Other duties that are specific to the office location and as assigned by Program Coordinator. Actual duties performed may vary widely according to the services offered by the office a staff person is assigned to.
  • Participate in monthly supervision meetings with Program Coordinator.
  • Provide general case management (including intake) and referral services to all residents needing such assistance.
  • Provide formal case management (i.e., evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident’s situation and needs) for a resident when such service is not available through the general community.
  • Develop and facilitate educational/support groups and complete statistics.
  • Establish linkages with all agencies and service providers in the community around to determine/develop the best "deals" in service pricing to assure individualized, flexible and creative services for the involved residents).
  • Set up a directory of providers for use by both project staff and residents.
  • Refer and link the residents of the project to service providers in the general community; these are, for example, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness and legal advocacy.

Job Type: Full-time

Pay: $20.00 - $22.50 per hour
