Overview
Charter Oak Health Center, Inc. (COHC) is a non-profit federally qualified health center (FQHC) located in Hartford, Connecticut with a long range vision of establishing a new wellness corridor known for comprehensive patient-centered services, highly qualified and dedicated team of health professionals, state of the art technology and collaborative partnerships.
 
We were founded in 1978 by residents of two public housing complexes in Hartford and over our 40 plus year history, have grown continuously to address the unmet primary health care and related needs of a city where about 60% of residents have incomes below the federal poverty level. We provide over 25,000 patients annually with primary medical, dental, behavioral health, some specialty, ancillary, enabling, and other services with about 100,200 encounters/visits with a team of 40 providers. COHC offers services at over 15 community-based locations in Hartford that are a combination of permanent and mobile van sites that serve the general community, school children, homeless individuals, and public housing residents.
 
We are a Patient Centered Medical Home model and provides “one-stop-shopping” for medical, dental, and behavioral health care with the goals of reducing health disparities and improving individual health outcomes.

Chief Executive Position
The Chief Executive works in partnership with a dedicated executive leadership team, oversees the overall operations for services supported by over 180 employees, build and maintain strong relationships with local and regional organizations, funders, and community and political leaders. The CEO is responsible for the organization’s leadership and for the achievement of the major strategic objectives under the direction of the Board of Directors.
 
The Center has an annual operating budget of about $21 Million. Agency revenues includes 67% patient revenue, 25% Federal grants, and 8% State and private funding.
 
The Board and Leadership Team have identified the following strategic priorities for the next 12-18 months to optimize what we do best, while enhancing services to the community. The specifics of a long-term vision and strategic plan that go beyond these priorities will be developed with the new CEO.
  • Provide a variety of high-quality integrated programs and services while being responsive to ever changing opportunities and challenges of community-based health care and ongoing implications of the Federal Health Care Act.
  • Attract, develop, and sustain dedicated staff, practitioners, and Board as well as enhancing our organizational systems for greater efficiency.
  • Develop and nurture viable collaborations with other organizations and build other services offerings to complement our business.
  • Strengthen the visibility, trust, and credibility of Charter Oak health services locally and regionally.
  • Sustain the profitability and diversify our revenue streams through maximizing funding opportunities, earned income, and fees.
The Ideal Candidate
We are seeking an experienced community health care leader who will inspire, guide, and advocate on behalf of Hartford community’s health care needs. He or she should have a proven record supporting an organization through a period of change and transformation, demonstrated leadership skills, strong communication skills, as well as being sound decision maker. The incoming CEO will bring strategic thinking and teambuilding to ensure quality programs to our community while focusing our strategic direction in response to changing needs. This is a full-time, exempt position based in Hartford, Connecticut.

 

 
 
Essential Qualifications and Experience
The ideal candidate will possess many of the following skills and experience:
  • Successful executive experience in a nonprofit or public federally qualified healthcare (FQHC) organization of similar size and scope, preferably in a CEO role.
  • Exceptional communication skills- including speaking, writing, meeting facilitation, and consensus building.
  • The ability to effectively represent Charter Oak and its programs and clients among the varied stakeholders in the community.
  • Financial and/or business management with a budget of comparable size and revenue streams, including budgeting, oversight, compliance and reporting abilities.
  • Ability to interact with clients and community members from diverse cultural, economic, and vocational backgrounds.
  • Experience with or an understanding of the local and regional issues, stakeholders, and trends of nonprofit, community based health care.
  • Experience effectively working and partnering with a community based Board of Directors.
  • Experience with local, federal and state compliance including HIPAA, OSHA, and FTCA.
  • Ability to conceptualize, communicate and lead the entire organization to shared visions and new directions, in response to changing community needs.
  • 10-12 years of supervisory experience at the Executive/Senior level.
  • Master’s Degree or higher in a relevant field.
In addition, candidates will need to:
  • Effectively utilize information technology and the Internet.
  • Work occasional nights and weekends as needed to support Board and community events. We offer flexibility to accommodate these obligations.
  • Provide proof of insurance and valid driver’s license.
  • Pass a background check.

Job Responsibilities
The following represents the incoming CEO’s role in partnership with a talented leadership team that includes the Chief of Operations, Chief Medical Officer, Chief Financial Officer, Chief Nursing Officer, Chief Human Resources Officer, Chief of Compliance & Legal Affairs, Vice President of Compliance and Chief Information Officer.
 
**Allocations provided as a guideline, given the organization’s priorities for the first 12 -18 months of the job.**
 
Management & Administration (35% of job responsibilities)
  • Maintain a climate that attracts, retains and motivates a diverse and high caliber team of staff members to achieve organizational performance.
  • Ensure that good human resource practices are in place, including up-to-date position descriptions and regular performance evaluations.
  • Ensure sound financial planning, management, reporting, and organizational accountability.
  • Oversee Charter Oak’s service delivery, risk management, and infrastructure (IT, policies, procedures, and facilities) to ensure compliance, client satisfaction, efficiency and effectiveness.
  • Ensure that all funds are disbursed according to contract requirements.
  • Negotiates all contracts and grants.
  • Ensure that the organization complies with all relevant laws, regulations, and applicable policies, including but not limited to HIPAA, OSHA and FTCA.
  • Coordination of reviews from regulatory funding entities such as HRSA and Joint Commission.
Community Relations and Partnerships (25% of job responsibilities)
  • Provide a credible and informed presence for Charter Oak in identifying and effectively addressing critical issues impacting community health and managed care.
  • Ensure that Charter Oak has effective two-way communication channels with government agencies, community leaders, clients, funders, and key collaborators to help establish its priorities and to effectively implement its programs.
  • Oversee the strategy, relationships, and content of government and foundation funding proposals and reports.
  • Ensure that funders, and other stakeholders receive timely communications on the organization’s activities and accomplishments.
  • Maintain membership in organization that further the mission and goals of the Center.
Strategic Direction and Evaluation (20% of job responsibilities)
  • Lead a learning organization dedicated to continual improvement and quality programs that meet client needs.
  • Develop and maintain a thorough knowledge of the issues and stakeholders surrounding community health care in the Hartford CT area.
  • Develop a process evaluating programs, services, and strategy to ensure consistency with the mission, values and priorities.
Partnership with Board of Directors (20% of job responsibilities)
  • Work with the Board of Directors to ensure long-range plans for the organization are in place, up to date, and used as a benchmark for organizational performance.
  • Ensure effective communications and linkages between Board and staff.
  • Serve as an active, non-voting member of Board and appropriate task forces.
  • Report regularly to the Board of Directors regarding trends, objectives, financial status of the organization, program effectiveness, client satisfaction, compliance, and other issues relevant to governance and oversight.
  • Assist the Board in identifying, recruiting, orienting, and utilizing Board members with the essential skills to support our mission and vision.
  • Successfully involve the Board in collaboration and outreach activities.
  • Ensure the implementation of Board policies and strategic priorities.
Ensure that administrative records such as minutes, bylaws are up to date and meet mandated licensing, and certification standards.
