Posting Summary
Grounded in our history as leaders in family-centered HIV health care, the François-Xavier Bagnoud Center (FXB Center) at the Rutgers School of Nursing serves communities that are at greatest risk for health inequities. The FXB Center works to achieve this mission through translation of findings to practice and policy, education, and research. The Community Outreach Worker is part of the HIV Clinical Practice team and is responsible for performing outreach to patients in the community and providing continuity of care by meeting patients in clinical care settings. Key job responsibilities include:
  • Maintain patient contacts required to assist patients to be engaged in regular health care services. This may include visits with patients at home, at health care facilities, and/or other locations to assist them in receiving services and to ensure that prescribed services are being received.
  • Provide emotional support for patients with respect to psychosocial aspects of HIV.
  • Initiate referrals needed to implement the case management plan.
  • Actively support and monitor patient linkages with services.
  • Keep accurate records of patient interactions. Contribute appropriate information to reports as requested.

Minimum Education and Experience

Required Knowledge, Skills, and Abilities
  • Four years of experience in community outreach work /case management assistance preferably involving community health related activities.
  • Bachelor’s Degree in social work or other related field may be substituted for the work experience. 
  • College credits may also be substituted for the work experience on a year-for-year basis.
  • Must be computer literate, specifically with Microsoft applications and database software. 
  • Ability to form and maintain relationships with people with HIV and their families. 
  • Valid driver’s license and access to a car.
Preferred Qualifications
  • Individuals with relevant personal life experiences are encouraged to apply.
  • Fluency in Spanish and/or Creole is desirable.

Immunization Requirements


Under Policy 60.1.35 Immunization Policy for Rutgers Employees and Prospective Employees , Rutgers University requires all prospective employees to provide proof that they are vaccinated against COVID-19 prior to commencement of employment, unless the University has granted the individual a medical or religious exemption. Rutgers University may require certain prospective employees to receive an updated (bivalent) vaccine dose. Under Policy 100.3.1 Immunization Policy for Covered Individuals , if employment will commence during Flu Season, Rutgers University may require certain prospective employees to provide proof that they are vaccinated against Seasonal Influenza for the current Flu Season, unless the University has granted the individual a medical or religious exemption.
