Summit Future Foundation is a rapidly growing nonprofit organization whose mission is to empower youth and young adults who experience developmental disabilities to lead a meaningful and inclusive life. We are currently seeking a full time Community Employment Supports Program Manager to join our team.

Home and Community Based Services Program Manager Role:

The Community Employment Supports Program Manager plays a crucial role in our organization's mission fulfillment efforts by assisting those we serve in obtaining and maintaining employment in their community. The Community Employment Supports Program Manager shall provide personnel oversight while demonstrating exceptional service delivery skills to the clients we support. The Community Employment Supports Program Manager works alongside the Executive Director and other members of the organizations leadership team to ensure that services are provided in a timely, person-centered, and professional manner while also adhering to quality assurance standards. This position provides an exciting opportunity for professional growth and development in a career as a disability services professional.

Community Employment Supports Program Manager Responsibilities:

Provide direct supervision of all program staff

  • Provide technical assistance to program staff in their implementation of clients service plan.
  • Development and oversight of program staff weekly schedules.
  • Oversight and review of staff’s daily documentation to ensure alignment with quality assurance standards.
  • Oversight, review, and dissemination of services monthly summaries.
  • Assist with on-boarding and training of new program staff.

Support agency and client services management

  • Design and develop client activities and tasks in their areas of employment readiness that will facilitate the achievement of employment and career related goals.
  • Utilize electronic information management system to organize and monitor client services and data.
  • Collaborate with department leadership to determine program staffing needs and communicate relevant client information to facilitate engagement in department’s service delivery.
  • Interface with case managers from targeted case management organizations to facilitate service evaluation and review.

Maintain a 50% FTE client caseload.

  • Provide person-centered and culturally competent services to your clients, in their home and their community, in a manner that supports them in reaching their employment goals while adhering to all organizations policies and procedures.

Required Skills and Experience:

  • Experience providing Job Coaching services to persons with developmental disabilities (minimum of three years).
  • Funder experience with Division of Developmental Disabilities and Vocational Rehabilitation.
  • Exceptional interpersonal and communication skills.
  • Ability to design tasks and activities that will result in persons we serve achieving their employment goals.
  • Demonstrate effective self management skills while leading others.
  • Ability to work independently and problem solve key issues.
  • Skilled in project and time management.
  • Tech savvy with experience in EMR and Google Workspace

Other Requirements:

  • Minimum education of High school diploma or equivalent.
  • US Citizen or Authorized to work in the United States
  • Valid Driver’s License
  • Reliable transportation with proof of current auto insurance and registration.
  • Ability to provide a clearance letter from the Family Safety Care Registry and pass a criminal background check.
  • Absence of a history of abuse and neglect to a person in a vulnerable population.
  • Current CPR & First Aid Certificate

Compensation & Benefits:
