$2,000 Sign-on Bonus!

After 6-month successful completion of probationary period

Full-time: 32 hours per week

Southeast Center for Independent Living (SCIL) is a non-profit organization that services individuals with disabilities in the Greater Fall River and New Bedford areas. We provide skills training, advocacy, information and referrals, transition and prevention services, and peer counseling to individuals who wish to become more independent in their life.

Summary

Options Counseling (OC) is person-centered, person-directed support, assisting individuals in need of long-term services and support to make informed choices about services that best meet their needs.

SCIL is seeking an Options Counselorto provide information and resources to the general community in respect to disability issues, emphasis on housing, transportation, health care, and personal care assistance. Provides residents with short-term interventions providing information about long-term services and supports that can make the difference between people remaining in their homes — or other preferred residential setting — or placement in a nursing facility.

Essential Duties and Responsibilities: (other duties may be assigned)

· Provide consumer-directed and interactive short-term decision-support to consumers, family members, caregivers and/or significant others in hospitals, nursing facilities, rehabilitation facilities, and community settings who are planning for long term services and supports.

· Gather information necessary to ensure services and support options provided and referrals made are viable and appropriate based on the consumer’s preferences, needs, and goals, as determined by the consumer.

· Develop a comprehensive and up to date resource database on community organizations, long term care services and support options, public and private payer resources, applications, etc.

· Assist the consumer in developing a plan that is consumer-directed and includes services, programs, financial resources, and other supports agreed upon by the consumer

· Complete and maintain documentation required using the Centers’ reporting on quality measures and outcomes, including contacts, progress notes, and surveys.

· Provide information and referral and service coordination between providers, family members and disabled individuals.

· Provide advocacy both individual and systems on issues impacting the disabled community such as housing, transportation, personal care assistance and health care.

· Meet with the Director of Programs and/or Executive at regular scheduled appointments for program progress.

· Attend scheduled staff meetings, in-service trainings, education trainings, and other regional collaborations with partners Work with staff involved in transition, prevention, and options counseling.

· Work closely with SCIL’s Director of Programs, Community Liaison and other staff to actively pursue and engage in outreach and marketing opportunities to promote Options Counseling.

· Track and gather data and information necessary to ensure required reports are accurate and complete.

Qualifications:

· Must have knowledge and/or experience working with individuals with a multitude of disabilities including behavioral health, substance abuse, cognitive, hearing and vision, and physical disabilities.

· Understanding of the commitment and philosophy of Independent Living and consumer direction. .

· Must possess good verbal and written communication, interpersonal, organizational, time management and follow-up skills.

· Must have valid driver’s license and reasonable transportation means.

· Must have computer knowledge/experience and ability to track and gather data for submission of reports in a timely manner.

· Must have knowledge of person centered transition planning.

· Must have knowledge of state agencies and the provider system.

· Bi-Lingual a plus

  • Must be fully vaccinated for COVID-19, unless can prove allowed exemption.

Education and/or Experience

Bachelor’s Degree is preferred in social work, human services, sociology, psychology, or related field. This requirement may be substituted for five years’ experience in human services.
