Program Coordinator, PC

 

 

Supervise the activities of Direct Support Providers to ensure adults with disabilities (clients) are provided a safe, healthy, and productive daily living environment. Identify appropriate internal and external resources necessary to meet client needs and ensure such services improve the client’s quality of life and meet Individual Service Plan (ISP) outcomes. Monitor residential settings to ensure compliance with licensing and Agency requirements.

  • Prepares and actively participates in ISP Schedules, facilitates and completes required documentation and teamings when appropriate.
  • Ensures all team assignments delegated by the ISP team are satisfactorily completed.
  • Identifies appropriate resources for provision of services in the areas of speech/physical/occupational therapies, psychological, psychiatric, educational, and vocational assessments, medical and dental needs, as well as any other service requested by the ISP Ensures direct care staff engages services for clients on a timely basis.
  • Reviews case records and evaluates performance of direct care staff members.
  • Provide direct service, informal counseling and support to clients as appropriate.

Job Type: Full-time

Pay: $41,600.00 per year

Education:

  • High school or equivalent (Required)

Experience:

  • Supervising Experience: 1 year (Required)

License/Certification:
