Community Health Worker for Sickle Cell Disease - San Diego
Cayenne Wellness Center and Children’s Foundation (CWC) is, a 501(c)3 organization dedicated to improving health outcomes and decreasing mortality related to Sickle Cell Disease (SCD). Our mission is to increase the quality of life for individuals diagnosed with SCD in California by ensuring expert, unbiased, and comprehensive care. This mission is part of a broader vision of (1) a medical system that effectively addresses the unique needs of the 7,000 residents living with SCD in California and (2) empowered patients who are equipped to advocate for themselves.

JOB SUMMARY:

Under the direction of the Executive Director and Program Manager, the CHW will (1) provide health education and health promotion using evidence-based information for SCD and related issues; (2) develop partnerships with family organizations to ensure that families are empowered as partners in their care; (3) assist families with obtaining SCD educational and social support services; (4) assist with transition services; and (5) engage in activities that help ensure individuals with SCD have access to services, including a medical home, from the time of diagnosis through the lifespan.

The job involves travel. You will be attending health fairs, support groups, hospital clinic hours, etc in an effort to get patients signed up and assessed.

The Community Health Worker (CHW) is responsible for coordinating access to services by assisting clients in utilizing available resources, including scheduling medical appointments, accompanying clients to appointments as needed, assisting clients with obtaining a medical home, providing instruction on appropriate use of the medical home, educating clients about available community resources and referring clients to local social service agencies, coaching clients on self-management, conducting home visits and communicating client related information back to the Care Coordination Team (CCT).

The CHW assists clients with the completion of medical program applications in order to build individual and community capacity and to serve as an advocate for clients and the community to assure they get needed services. The Community Health Worker is also responsible for identifying clients, completing initial needs assessments, and maintaining follow-up with clients to ensure appropriate guidance to health services.

Employment Type: Full and Part-Time Hours/Per Pay Period: 80-160

CORE DUTIES: The CHW is responsible for identifying patients diagnosed with SCD (approximately three persons/month), and empowering them by ensuring they receive appropriate follow-up services, providing social support services, educating on FDA SCD-approved treatment, and assisting emerging adults in the transition from pediatric to adult-focused health care (if applicable). They work with hospitals and medical centers, state agencies providing newborn screening follow-up, hospitals, schools, churches, and other CBOs that provide services to patients with SCD.

QUALIFICATIONS: *
*Minimum Qualifications (REQUIRED): B.A. degree, 2 years experience in health, social, or community services, CA driver’s license (proof of auto insurance required)

Preferred Qualifications: Bilingual Spanish-English preferred; Previous experience working in/with local CBOs or agencies, B.A. or up, Nurse, Nurse Practitioner, or Social Worker.

Previous experience working in/with local community-based organizations or agencies is preferred. Training will be provided if you are not a Certified Community Health Worker.

Job Type: Full-time

Pay: $19.00 per hour
